Medicare Plus and Enhanced Primary Care Plans
Claire is registered with the Health Insurance Commission and available to participate in multidisciplinary teams on Enhanced Primary Care Plans. We can assess your patient's current nutritional status and provide medical nutrition therapy and education of patients, carers and families. You will be sent a report on your patients progress after each consultation.
